If you need a template to use to prepare your Invitation Letter, you are welcome to use this one.

- Go under File to Make a copy.
- Fill in your own information.
Regardless of what template you use, please print your document and complete it (but do not sign it) before your meeting with the Notary. Or, if you want the Notary to print it for a fee of $1.50 per page, email it to [email protected] before your meeting so it can be printed and ready for you to sign when you arrive.
Also, remember that whoever signs will need one piece of government-issued, non-expired, photo ID. Whoever signs the letter MUST be physically present so the Notary can see them sign the document in person.
